Thank you for your interest in making a gift of stock! Please share this form with your broker so that they have all needed information to process the transaction and ensure that you get acknowledged for the gift.
When making stock transfers, it is very important that the broker contact Virginia MOCA to inform us of which donor they represent, the amount of the transfer, the date of the transfer, and the type and name of the stock being sold so we can give proper acknowledgment to the donor.
